A manuscript typically becomes sharper and shorter during the editorial process; on the other hand, substantive queries can lead to paragraphs and even pages of new material.
It is very helpful to know that you want these services when we start so my brain can begin functioning in that area as well. Indexing and bibliography work includes the insertion of codes in a manuscript, and knowing that you want that done is most efficiently performed the first time through (and it can be changed later with less effort than adding the bibliography/index at the end, particularly if deadlines are tight).
